Registration Form / Liability Waiver
All registration for programs, activities, and special events sponsored and/or co-sponsored by the City of Paso Robles Department of Library and Recreation Services shall be done at the registration office.

Registration Hours:
Office hours at Centennial Park registration office are from 9:00 am to 5:30 pm, Monday through Friday, except posted holidays. Registration can be accomplished by the following methods:
Walk-in
Registration:
Payment at the Centennial Park Registration desk between the
office hours of 9am to 5:30pm, Monday through Friday. Prior
notification will be posted when office will be closed for the
holidays.
Mail Drop System:
Before or after hours, when the door is open to the Centennial
Park lobby, our mail drop slot in the registration door can be
utilized. Completion of the
registration form is
required and ONLY CHECKS (no cash, please) made payable to the
City of Paso Robles.
FAX in
Registration:

Fax registration form with
a credit card number and signature (minimum $10).
Mail-in
Registration:
Check or money order sent along with a
registration form
filled out with all required information.
