INCIDENT REPORT REQUESTS

You may download and digitally fill out the Incident Report Request Form, but you must print it and include an original signature before delivering or mailing to the Emergency Services Department at 900 Park Street, Paso Robles CA 93446.

A $15.00 check made payable to "City of Paso Robles" must also be included. Requests are normally processed within 5 business days.
